Charis Healthcare provides virtual and in-person medical care services including but not limited to primary care, chronic disease management, metabolic health and weight loss support, behavioral wellness, alcohol reduction support, mobile wound care, and DOT physical examinations. Services are provided by licensed healthcare professionals and are subject to clinical appropriateness and state licensure requirements.
Virtual care services are available only in states where Charis Healthcare providers are licensed to practice. Service availability may change without prior notice.
CHARIS HEALTHCARE SERVICES ARE NOT INTENDED FOR MEDICAL EMERGENCIES. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this website or attempt to contact Charis Healthcare in an emergency situation.

Use our services only for lawful purposes and in accordance with these terms

By scheduling and attending a virtual visit with Charis Healthcare, you acknowledge and consent to the use of telehealth technology for the delivery of healthcare services. You understand that:
Charis Club membership is a subscription-based program billed monthly. Membership fees are non-refundable for the current billing period. Members may cancel at any time by contacting Charis Healthcare at services@charishc.com or (470) 604-9800.
Cancellation takes effect at the end of the current billing cycle. Membership does not replace health insurance and does not cover all services.
Payment for services is due at the time of service. Charis Healthcare accepts most major insurance plans for eligible services. Insurance coverage is not guaranteed, and patients are responsible for any amounts not covered by their insurance plan. Self-pay pricing is available — please contact our office for current rates.
Your health information is protected in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and our Privacy Policy. Please review our HIPAA Notice of Privacy Practices for full details on how we collect, use, and protect your information.
All content on the Charis Healthcare website, including text, graphics, logos, and images, is the property of Charis Healthcare and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from our content without express written permission.
To the fullest extent permitted by law, Charis Healthcare shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services or website. Our total liability for any claim shall not exceed the amount paid by you for the specific service giving rise to the claim.

These Terms & Conditions are governed by the laws of the State of Georgia. Any disputes arising from these terms or your use of Charis Healthcare services shall be resolved in the courts of Cobb County, Georgia.
